☐ Completed general application
☐ Non-refundable application fee of $150.00, or $300 per family
☐ Copy of Birth Certificate (Mother’s Copy is not accepted.)
☐ Immunization record (REMINDER: All students may not
begin school
until immunization record record or religious exemption letter is received.)
☐ Copy of last year’s final report card, plus current year’s grades
☐ Copy of last year’s standardized achievement test scores
☐ Completed Pastoral Reference form
☐ Completed Teacher Reference form (Grades 1 – 7)
☐ Admissions testing by appointment (Grades K-7) Prospective student will be
notified by Admissions to schedule appointment.
☐ Parent Interview with Board Member and/or school administrator (All Applicants)
Parent interview will be notified by Admissions Office regarding date and time.
Grace Christian Academy admits students of all races, color, nationality, gender and ethnic origin.
Upon completion of required interviews, tuition contracts will be offered to parents. Remember applicants will be considered ACTIVE students when the contract is returned.
